A clinical relationship begins only after applicable intake, consent, eligibility, and evaluation requirements are completed and Frontier Diabetes accepts the individual for care.

Frontier Diabetes does not provide emergency or continuously monitored care. Do not use this website, email, voicemail, or the patient portal for an emergency. Call 911 or go to the nearest emergency department. For an urgent concern that is not an emergency, contact an appropriate local clinician or urgent care center.

Frontier Diabetes currently plans to provide telehealth services to eligible patients who are physically located in Idaho at the time of care. Telehealth may not be appropriate for every concern. In-person examination, laboratory testing, emergency services, or referral to another clinician may be recommended.

Frontier Diabetes works to keep website information useful and current, but does not guarantee that all content is complete, error-free, or continuously available. Services, pricing, scheduling, eligibility, and website content may change. No particular clinical outcome is promised or guaranteed.
