This website privacy policy is separate from the HIPAA Notice of Privacy Practices provided to patients through the Frontier Diabetes intake process. Clinical records and protected health information created or maintained as part of patient care are handled under that Notice of Privacy Practices and applicable law.
Information you choose to provide
You may choose to contact Frontier Diabetes by phone, text, or email with general questions about services, pricing, scheduling, or becoming a patient. Please do not send diagnoses, medication lists, glucose reports, laboratory results, refill requests, photographs, or other private medical information through ordinary email, voicemail, text message, or public website features.
Established patients should use the secure patient portal for clinical communication.
Information collected automatically
The website host may process limited technical information needed to deliver and protect the site, such as an Internet Protocol address, browser or device type, requested pages, approximate location derived from an IP address, timestamps, and security logs. Frontier Diabetes does not currently use Google Analytics on this website. This policy will be updated if analytics or additional tracking technologies are added.
How information may be used
Information may be used to respond to general inquiries, operate and secure the website, diagnose technical problems, understand site performance, comply with legal obligations, and protect the rights and safety of Frontier Diabetes and its visitors. Frontier Diabetes does not sell personal information.

Frontier Diabetes may use service providers for website hosting, secure scheduling, patient-portal access, communications, and payment processing. Those services may have their own privacy practices. Links to third-party websites do not mean Frontier Diabetes controls or is responsible for their content or privacy practices.
