Every patient starts with a complimentary 15-minute introductory call. Together, we’ll review your medications and biggest concerns, then help you choose the right level of care.

Use your health plan for eligible visits.

Frontier Diabetes will accept Idaho Medicaid and will be in network with all Blue Cross Blue Shield plans, including Medicare Advantage. Participation with additional health plans is in progress.

Quarterly Support

For stable patients who primarily need quarterly follow-up.

  • One scheduled 30 minute diabetes visit
  • Medication, glucose, CGM, pump, and lab review as relevant
  • Routine refills and prior-authorization support
  • Secure messages for simple questions
  • Help coordinating routine medication and supply issues

A lower-cost maintenance option for patients who do not need monthly follow-up.

Temporary add-on

Intensive Support

+$100per month

Added to Ongoing Support; $199/month total.

For patients who need frequent medication changes, glucose review, insulin adjustment, or close follow-up.

  • Everything included with Ongoing Support
  • More frequent follow-up while treatment is being adjusted
  • Frequent glucose, CGM, or insulin review as needed
  • Medication adjustment and prescribing when appropriate
  • Additional help during insulin initiation, pump transitions, or other unstable periods

Return to $99/month Ongoing Support when intensive follow-up is no longer needed.

Standalone option

Medication Refill Visit

For new or established patients who are stable, know what they need, and do not need support between visits.

  • One scheduled 15 minute video appointment
  • Review of your current medication and relevant health history
  • Prescription renewal when clinically appropriate

New symptoms, significant medication changes, or more detailed diabetes management may require a Comprehensive Initial Visit. Recent records or laboratory results may be requested before a prescription is renewed.
